Out Of Home Care Manager

Our CareChoice team are passionate about their work and pride themselves in delivering an exceptional calibre of service. As a result of continued growth and expansion Carechoice is seeking an experienced Out of Home Care Manager to oversee a portfolio of clients in the Barwon, Western and Northern regions of Melbourne. The RoleThe role of the Out of Home Care Manager is most integral to the delivery of a high level of quality supports for individuals with complex needs.  Excellent leadership, decision making, rapport building, problem solving, negotiation, conflict resolution, organisational skills and genuine passion are critical to the success of the role. You will be responsible for:Operational Management of OOHC Team Leaders who are providing coordination of supports to complex client programs.Regular assessments of quality of care and program assessment (spot checks).Manage and participate in the CP on call response to ensure appropriate coverage.Ensuring program is on track to deliver real outcomes in terms of goal attainment for clients.Ensuring the quality of supports is in line with allied health partner, CP Practitioners, Support Coordinator / Case Manager recommendations.Oversight of progress notes and all reporting requirements on allocated programs.Crisis management and Incident reporting follow up.Facilitating regular team meetings, both with internal and with regional stakeholders.Your ProfileTo be successful in the position you must have:Proven experience in a customer focused Community Services environment.Proven experience managing people and programs in the Disability, Youth Welfare, Child Protection Out of Home Care and/or Community Services Sectors.Proven experience in a position driven by customer outcomes.Strong networking and relationship management skills.Proven ability to think and plan strategically with excellent analytical and problem solving skills.Proven ability and strong desire to ensure clients are in receipt of exceptional service standards.Well developed interpersonal, negotiation and complaints resolution skills.Sound verbal and written communication skills.  AUD Australia - VIC Melbourne - All locations Bayside & South Eastern Suburbs Clayton 3168
