Support Coordinator

As a result of continued growth we are now seeking an experienced Support Coordinator based in our Clayton office to manage a port folio of clients programs across MelbourneThe RoleThe role of the Support Coordinator is most integral to the delivery of a high level of quality supports for individuals with complex needs. You will provide individualised plan implementation and support to people with disability. Excellent leadership, decision making, rapport building, problem solving, negotiation, conflict resolution, organisational skills and genuine passion are critical to the success of the role. The principal purpose of the role is to ensure that service users are completely satisfied with the service delivery, that complex programs are delivered with a high level of support worker to client engagement and clients are on track to achieve their goals as articulated in their NDIS plan or Care Plans. Your key responsibilities will include but not be limited to:Providing Coordination of Supports to clients as per allocated portfolio under NDISRegular assessments of quality of care and delivery of servicesEnsuring program is on track to deliver real outcomes in terms of goal attainment for clientsEnsuring the quality of supports is in line with best practice and in line with Allied health partner, Support Coordinator / Case Manager recommendationsOversight of progress notes and all reporting requirements on complex programsCrisis management and Incident reporting follow up where requiredFacilitating and participating in regular care/clinical team meetings Participate in the On-Call rosterFrom time to time, this position may be assigned a particular project or an additional ongoing responsibility. This will require planning, organisation and co-operation in order to achieve our organisational objectives Your ProfileTo be successful in this position it is essential you have:Previous experience in a similar position within the support worker industry and a sound understanding of the models of support for people with disabilityDemonstrated experience in support planning, working within a person centred human rights framework Relevant tertiary qualificationsAbility to build rapport with clients and effectively communicate with various people from industry organisationsExcellent problem-solving skills and creativity.Excellent written and communication skillsExcellent computer literacy and high level of administration skillsCurrent police checkCurrent drivers licenseA background in Nursing or in any allied health profession would be highly regarded.
