This newly created role will focus on providing clinical expertise regarding the support needs for Community Aged Care clients receiving in-home supports.The Team Leader will also support the Business Development Manager and Client Engagement Managers (CEMs) to identify appropriate clients with appropriate levels of need.This role will also be the point of escalation for any client-related incidents that require clinical intervention or direction for Personal Care Attendants (PCA’s).A day in the life as our Aged Care Team Leader, some of your key responsibilities will include:Supporting CEMs to identify services that will best meet the client’s needs.Reviewing and endorsing client Support Plans.Monitoring and reviewing client’s needs and effectiveness of Support Plans.Supervising and supporting work practices of relevant direct care staff to ensure optimum standards of service delivery.Liaison with the Coordination team to ensure appropriate allocation of PCA’s to client rosters of care.Facilitating regular aged care team meetings with direct care staff.Facilitating appropriate client-specific training for direct care staff.Developing an extensive knowledge of services/resources available within the community.About youAs our Team Leader – Aged Care, you will have strong attention to detail and be a highly effective decision maker. In addition, you will have a pro-active attitude and a positive approach to achieving outstanding outcomes for our clients.You will be self-motivated, amiable, honest, perform the role with the highest level of professionalism and integrity, with a genuine customer service focus.Ideally, you’ll have:Demonstrated knowledge of the aged care sector, aged care legislative standards, and guidelinesRegistered Nurse Division 1 or Enrolled Nurse Division 2 (with Medication Endorsement) with current AHPRA RegistrationFirst Aid Certificate – Level 2Demonstrated experience in assessment and case management in community aged careDemonstrated experience and understanding of the OHS issues associated with the provision of care services in the home and community contextSound knowledge of issues associated with ageing, particularly for clients who wish to remain in their own homeAble to work with people who have complex support needs to ensure an effective and ongoing relationshipCurrent Victorian Drivers LicenceEffective communication skillsAbility to work with family members in situations which may involve conflict, anxiety, or stressStrong time management skills – including the ability to set priorities and meet deadlinesStaff management skillsFinancial and business management skills; Employment with CareChoice is subject to satisfactory background checks which include Professional References, Employee Working with Children Check, Disability Worker Exclusion Scheme Check and NDIS Worker Screening Clearance. Employment with CareChoice is also subject to satisfactory evidence of COVID-19 Vaccination (including Booster shot).
